Baby boy Jacob Densiel Perez-Ostolaza, beloved son of Dennys Jhym Perez Avalos and Suleyka Ostolaza Rolon, entered eternal rest on November 4, 2016 at Bridgeport Hospital. Jacob is survived by: paternal grandparents, Tomas Hinostroza and Mery Perez; maternal grandparents, Joselin Ostolaza and Zoraida Rolon; aunts, Aracely Hinostroza, Lisandra Ostolaza; cousins, Francis Perez, Brenda Rondon, Stephane Velez, Zorilys Ostolaza, Zashely Perez, and a host of extended family and friends. Interment will be held Thursday, November 10, 2016 at 10:00 AM at St. Michael's Cemetery, 2206 Stratford Ave. Stratford, CT 06615.
